Hereby apply for full/associate * membership of the Construction Health and Safety Group and, if elected, agree to abide by the Memorandum and Articles of Association and any regulations or rules for the time being regulating the affairs of the group.

As members, £100 of your membership subscription is redeemable against the cost of any one-day course in the Group’s prospectus booked during this subscription year. This training benefit is redeemable up to 30th April 2011.
We agree to be contacted and provide methods of payment for £223.38 inc VAT for the annual subscription for the current year ending 30th April.
